What the Latest Research Says About Creatine and Your Brain

For thirty years, creatine has lived almost entirely in the gym. It's the powder in the shaker bottle, the supplement lifters swear by, the thing associated with bigger squats and faster sprints. All of that is well earned. Creatine monohydrate is one of the most studied, most reliable performance supplements in existence. But over the past few years, the science has taken a genuinely interesting turn. Researchers have started asking a different question: if creatine fuels hard working muscle, what does it do for the hardest working organ of all?
The brain is an energy hog. Despite being about 2% of your body weight, it burns roughly 20% of your energy. And it runs on the same fundamental currency your muscles do, ATP, the molecule cells use for quick energy. Creatine's job in the body is to help regenerate ATP rapidly, keeping a reserve tank topped up for when demand spikes. That's exactly why the brain health question is so compelling. Let's walk through what the most recent research actually shows.
The Core Idea: Creatine Is a Fuel Reserve, Not an Engine Upgrade
Before the specifics, it helps to understand the single most useful framing that has emerged from recent reviews. Creatine doesn't make a healthy, well rested brain smarter. It deepens the fuel reserve so your brain can keep performing when its energy supply is under strain.
This distinction explains almost every finding below. The effects in rested, omnivorous, healthy young adults are small to nil, because creatine tops up a tank that's already full.
But when the tank is running low, whether from sleep loss, high mental demand, low dietary intake, aging, or illness, that reserve suddenly matters a great deal. As one 2026 review put it, individuals exposed to increased metabolic stress or with existing brain creatine deficits, such as from sleep deprivation or neurological disease, may be particularly responsive to supplementation [1].
Keep that in mind as you read. Creatine's brain benefits aren't universal. They're concentrated in exactly the situations where your brain is short on energy.

One recent review summarized the emerging view well, noting that the depression evidence is genuinely promising, with several trials finding creatine accelerated and amplified SSRI response, especially in women [8].
To be clear, this is adjunctive. Creatine is a potential complement to established treatment, not a replacement for therapy or medication. But as a cheap, safe add on, the signal is real.
Cognition, Memory, and Aging
For everyday cognitive function, the picture is more modest and more nuanced, which is exactly what the fuel reserve framing predicts.
A widely cited meta analysis concluded that creatine monohydrate supplementation may confer beneficial effects on cognitive function in adults, particularly in memory, attention, and information processing speed [9]. The confidence isn't uniform across every domain, though. In that analysis, the certainty of evidence for memory was moderate, while evidence for processing speed, executive function, and attention was lower, indicating that further research is needed [9].
Two groups tend to benefit most, both consistent with the idea of topping up a genuinely low tank. Vegetarians show some of the largest and most reliable memory gains in the literature, consistent with topping up low stores, which makes sense since dietary creatine comes mainly from meat and fish [8]. And older adults show modest memory improvements, an active research area alongside creatine's muscle preservation benefits [10].
The Frontier: Neurological Disease
The most preliminary but most intriguing research is in neurological conditions where brain energy metabolism is impaired. Reviews point to potential in areas like traumatic brain injury and dementia, precisely because these involve the kind of metabolic stress creatine seems built to buffer [1].
This work is genuinely early stage, with modest effects, small samples, and many open questions, and no one should read it as a treatment recommendation. But it's an active, well funded area precisely because the underlying logic is sound: restore the brain's energy reserve, and you may support its resilience.
What This Means Practically
If you're considering creatine for your brain rather than your bench press, a few honest takeaways from the current research:
The standard dose is the same. Most studies use 5 grams a day of creatine monohydrate, the same cheap, well tolerated form used for decades in sports. Some brain and sleep deprivation studies have used larger acute doses, but 5 grams daily is the sensible baseline. Notably, some researchers now argue that more may be better for brain bioenergetics, since higher doses have shown added increases in brain creatine levels, though this is still being worked out [11].
It's remarkably safe. Creatine is one of the most studied supplements on the market. The main consistently reported side effect is slight weight gain from water retention. Still, check with your doctor, especially if you have kidney concerns or take other medications.
Set realistic expectations. If you're a well rested person eating plenty of meat, don't expect a cognitive transformation, since your tank is likely full. The benefits concentrate in the stressed, the sleep deprived, the plant based, the aging, and those managing mood conditions.
It's not a fix for the big levers. As one review sensibly noted, if you have persistent brain fog, fatigue, or low mood, the high yield move is medical evaluation, meaning bloodwork, a sleep history, and a proper screen, not a tub of powder. Creatine, at its best, is a few grams of help for a system whose bigger levers still do the heavy lifting [8].
The Bottom Line
Creatine's second act is one of the more genuinely exciting stories in nutrition science right now. The evidence has moved well beyond the gym, and the throughline is elegant. The same molecule that helps your muscles power through a final rep may help your brain power through a sleepless night, a low mood, or the metabolic stresses of aging. The research is still young, and honest scientists are quick to flag the gaps. But the direction is clear, the mechanism makes sense, and the risk to reward ratio, for a cheap, safe, decades tested supplement, is hard to argue with.
The engine is your brain. Creatine just makes sure the fuel reserve is deep enough for when the road gets hard.
