PRP Therapy: What the Research Actually Shows Across Its Most Popular Uses

Platelet-rich plasma, or PRP, has quietly become one of the most requested regenerative treatments in medicine, and for good reason. It's drawn from your own blood, which makes it remarkably safe, and it's being used for everything from thinning hair to aching knees to aging skin.


But "used for everything" and "proven for everything" aren't the same thing. The evidence behind PRP is genuinely strong in some areas, promising in others, and thin in a few places where it's heavily marketed. Here's an honest look at where PRP earns its reputation, based on the most recent research.


First, what PRP actually is


Your blood contains platelets, and platelets are packed with growth factors, the signaling molecules your body uses to repair tissue. To make PRP, we draw a small amount of your blood, spin it in a centrifuge to concentrate the platelets, and then place that concentrated plasma exactly where healing is needed.


The concept is simple and elegant: take your body's own repair signals, concentrate them, and deliver them to a spot that needs help. Because it comes from you, there's no risk of rejection or allergic reaction, and side effects are typically minor and short-lived.


One thing to hold onto as you read: PRP is not one standardized product. How the blood is processed, how concentrated the platelets are, and whether the platelets are "activated" all vary between clinics, and those differences meaningfully affect results. More on that at the end, because it matters more than most people realize.


1. Hair restoration (androgenetic alopecia)


This is one of PRP's best-supported uses, and the evidence keeps getting stronger.

A large 2025 systematic review and meta-analysis pulled together 43 randomized controlled trials covering nearly 1,900 patients. The conclusion: PRP is safe and effective for hair loss, increasing hair density and improving patient satisfaction compared with placebo. Notably, the analysis found that activated PRP performed better, a good example of how protocol details drive outcomes.


Where PRP shines is early-to-moderate pattern hair loss, in people whose follicles are still alive but weakened. When injected into the scalp, the growth factors appear to reawaken those struggling follicles. It works less well, or not at all, for completely bald areas or scarring types of hair loss where the follicles are already gone.


It also pairs well with existing treatments. Research suggests PRP combined with minoxidil tends to outperform either one used alone.


Worth knowing: PRP for hair loss is technically an off-label use. That's a regulatory classification, not a safety warning, but it's the honest context.


2. Joint and musculoskeletal pain (especially knee osteoarthritis)


If hair loss is PRP's aesthetic flagship, knee osteoarthritis is its orthopedic one.


A 2025 meta-analysis of randomized trials found that PRP injections produce a clinically significant improvement in knee osteoarthritis, and that the degree of benefit is influenced by how concentrated the platelets are. Other pooled analyses comparing PRP against hyaluronic acid (a long-standing joint injection) have found that PRP tends to provide better pain relief at the 6- and 12-month marks, meaning its advantage shows up in longer-term durability rather than instant results.


The best candidates are people with mild-to-moderate osteoarthritis. In advanced, bone-on-bone joints, PRP is far less likely to help. Beyond the knee, PRP is also widely used for tendon problems like tennis elbow and other overuse injuries, though the evidence there is more variable and depends heavily on the specific condition.


The appeal for the right patient is real: a non-surgical, drug-free option that uses your own biology, with a strong safety profile.


3. Facial skin rejuvenation (often with microneedling)


You may have heard of the "vampire facial." Underneath the marketing name is a legitimate combination: microneedling paired with PRP.


Microneedling creates thousands of tiny channels in the skin that trigger a collagen-building response and, conveniently, help the PRP penetrate. Applied or injected alongside microneedling, PRP's growth factors appear to enhance that repair process. Studies have shown improvements in skin texture, tone, and elasticity, and adding PRP to microneedling has been found to improve atrophic acne scars more than microneedling alone, with rolling and boxcar scars responding best.


Be aware that this is where the honest caveat gets bigger. Many of the facial-rejuvenation studies are small, and response tends to be age-dependent, with results building gradually over multiple sessions rather than appearing overnight. It's a legitimate treatment with encouraging data, but not a miracle, and expectations should be set accordingly.


A note on other marketed uses


PRP is also promoted for sexual wellness (the "P-Shot" and "O-Shot") and various other applications. These are popular, but the high-quality clinical evidence behind them is considerably thinner than for hair, joints, and skin. That doesn't mean they can't help anyone; it means the science hasn't caught up to the marketing yet, and any honest provider will tell you so.


The thing that actually determines your results


Here's the detail that separates a good PRP outcome from a disappointing one, and it's rarely advertised: the preparation matters as much as the procedure.


PRP is not a single, standardized drug. Platelet concentration, whether the PRP is activated, the number of sessions, and where and how it's injected all influence whether you get a meaningful result. The 2025 knee osteoarthritis analysis found benefit was tied to platelet concentration. The hair-loss analysis found activated PRP outperformed non-activated. In other words, two clinics can both offer "PRP" and deliver very different outcomes.


This is why PRP should be thought of as a medical treatment guided by protocol and candidacy, not a menu item. The right question isn't just "do you offer PRP?" It's "is PRP appropriate for my specific situation, and is it being prepared and delivered in a way the evidence actually supports?"


The bottom line


PRP is one of the more scientifically grounded regenerative treatments available, with genuinely strong evidence for hair restoration and knee osteoarthritis, promising evidence for skin rejuvenation, and a safety profile that's hard to beat because it comes from your own body.


But it isn't magic, and it isn't one-size-fits-all. The best results come from matching the right patient to the right application, with a preparation protocol that reflects what the research shows works. Done thoughtfully, it's a powerful tool. Done carelessly, it's an expensive placebo.
